Sign on Bonus!! $15,000 (over 3 years experience) $10,000 (less than 3 years experience) The Occupational Therapist evaluates residents, develops individualized care plans, and provides therapeutic interventions as prescribed by the referring physician, taking into account each resident's physical, cognitive, and age-related needs, in order to help residents achieve their highest practicable level of function and independence within the skilled nursing facility setting.
ESSENTIAL JOB FUNCTIONS 1.
Determines the level of function and needs of all referred residents by performing timely and appropriate evaluations. 2. Establishes goals and a plan of treatment, including the resident's own goals, by preparing a written treatment plan that is placed in the resident's medical record. 3. Adheres to the interdisciplinary model of care by integrating Occupational Therapy interventions into the total resident care plan, consulting with the physician, family, nursing staff, and other members of the care team as needed. 4. Ensures resident well-being by carefully preparing the resident for treatment and administering appropriate interventions, remaining mindful of all precautions, indications, and contraindications. 5. Provides intervention by following the established treatment plan and documents the resident's response to treatment in a progress note after each session. 6. Stays apprised of changes in a resident's condition through ongoing reassessment, revising the treatment plan and goals as indicated and communicating the resident's status to the physician and interdisciplinary team. 7. Supports continuity of care by instructing residents, families, and caregivers so they can provide follow-through with therapy programs after discharge. 8. Assures record-keeping and documentation accuracy, including: writing clear, concise, and timely evaluations and progress notes; completing discharge summaries with the reason for discharge and recommendations; providing required productivity/utilization reports (e.g., residents referred and discharged, treatments provided, hours of service); and completing charge entry, MDS-related documentation, and insurance/billing paperwork according to facility policy. 9. Assists the Rehabilitation Manager by evaluating the need for Occupational Therapy equipment and supplies. 10. Interacts with other Rehabilitation staff by attending team/care-plan meetings and communicating resident needs to other involved therapists; co-treats residents with other disciplines as indicated. 11. Assures a safe and clean Occupational Therapy treatment area by overseeing the care of related equipment and the environment. 12. Assures quality of care by using approved procedures and facility/regulatory standards for the administration of all Occupational Therapy services. 13. Maintains a high level of quality assurance for the Rehabilitation Department by keeping current on new developments in Occupational Therapy practice; attends required in-services and completes competency requirements.
QUALIFICATIONS
Education:
Bachelor of Science (or higher) degree in Occupational Therapy from an accredited program, and certification by the National Board for Certification in Occupational Therapy (NBCOT).

Registration:
Current, valid state Occupational Therapist license in good standing.
Experience:
Minimum of two (2) years of experience as an Occupational Therapist preferred; geriatric or skilled nursing/long-term care experience preferred.
Knowledge:
Working knowledge of state and federal regulations pertaining to Occupational Therapy and skilled nursing facility care (including applicable CMS/MDS requirements); knowledge necessary to provide care appropriate to the age of the residents served; knowledge of the principles of growth and development across the life span, including geriatric-specific needs. Abilities Ability to perform the essential functions of the job. Possesses the skills necessary to provide care appropriate to the age of the residents served, particularly older adults. Ability to assess data reflective of a resident's status and interpret the appropriate information needed to identify each resident's requirements relative to their age-specific needs. Ability to provide care as described in the facility's and department's policies and procedures. Ability to demonstrate knowledge of the principles of growth and development over the life span, assess resident status data, and interpret information needed to identify age-specific care requirements.
