In this role, you will work directly with clients in home, clinic, and community settings to deliver goal-directed, skill-building interventions tailored to their Individualized Rehabilitation Plans (IRPs). You will collaborate closely with JTC's clinical team, licensed therapists, external psychiatrists, and community partners to ensure wrap-around support and regulatory compliance Responsibilities Provide face-to-face individual and group sessions focused on independent living skills, emotional regulation, crisis de-escalation, budgeting, social skill development, and wellness. Implement structured activities aligned directly with the client's Individualized Rehabilitation Plan (IRP) as established by the clinical team. Accompany clients into community settings to practice social skills, access local resources, navigate public transit, and attend necessary medical or mental health appointments. Complete concurrent, high-quality progress notes and contact logs in the agency's EHR system. Maintain regular communication with the client's primary therapist, prescribing psychiatrist, school system (for youth), and family members (when authorized) to track progress and identify emerging needs. Participate in weekly/monthly clinical supervision, team meetings, and mandatory continuing education modules.

Experience Required:

Education:

High School Diploma or GED with 2+ years of direct behavioral health experience.

Transport Requirements:

Valid Driver's License, active personal auto insurance, clean driving record, and a reliable personal vehicle suitable for community visits and client transport.

Clearances:

Ability to pass a comprehensive state criminal background check, FBI fingerprinting, and protective service background checks.

Preferred:

Bachelor's degree in Psychology, Social Work, Counseling, or Human Services preferred. Experience with Maryland Medicaid regulations, COMAR guidelines, and Optum authorization workflows. CPR/First Aid Certification.

Experience:

Minimum of 1 year working with individuals diagnosed with Severe Mental Illness (SMI) or Emotional/Behavioral Disorders (EBD).

Technology:
