This position will have telephonic contact with patients, family members and DOHC staff. The Urology Liaison will be responsible for facilitating full circle care plans focused on the patient's well being and care. Professional Duties Provides coordination to high-risk Urology members. Responsible for coordinating certain operations including collaboration of health care team that can include physicians, pharmacists, nursing staff, social workers, home health staff and other services offered by the medical group to facilitate an effective, comprehensive and patient-centered plan. Be a customer service oriented individual who will facilitate effective communication between patient, their representatives and entire DOHC network. Assists in obtaining relevant supporting medical documentation and creating, completing, revising and submitting referrals to applicable medical specialties, provider and affiliated DOHC services (i.e. home health, palliative care, Family Hospice). Interviews patients and families to assist and identify potential barrier hindering patient care such as financial resources or other social determinants of health, emotional state, or adequate support system and will communicate specific findings to the appropriate DOHC programs or case management teams who will be participating in the resolution of the barriers to patient care. Organizes, prioritized and implements daily tasks related to the timely access of member care, facilitates and coordinates urgent consults and appointments, assists with the completion of diagnostic tests while assuring smooth transition of care. Document and communicate effectively problem, intervention and plan with EMR. Be an ambassador of DOHC and exhibits professionalism in all daily functions, uses the language of caring in all interactions and observes proper telephone etiquette. Other duties as assigned. Qualifications High school diploma or equivalent required (some college education preferred but not necessary) Minimum 2 years previous experience in the medical field (case care coordination, referral experience preferred but not required) Familiarity with medical terminology Strong computer skills required Must possess excellent telephone etiquette, and interpersonal communication skills with an ability to interface with the case management team, physicians, DOHC staff and outpatient providers in a pleasant and professional manner. Must be detail oriented and able to continually re-prioritize multiple tasks Bilingual preferred At the discretion of

management, this position has the potential to be a full or hybrid telecommuting position.
