At NIVA Health, we're looking for a Wellness Case Manager to help guide patients through their wellness journey — from their first interaction with our clinic through ongoing programme engagement. You'll work closely with providers and clinic leadership to help patients stay engaged, informed, and connected to the services designed to improve their overall wellness experience.
