FTE JOB DUTIES

This position is located in the Cancer Center unit of Kona Community Hospital. The Cancer Center provides oncology services for the island/county of Hawaii, including consultation, chemotherapy and immunotherapy administration, infusion therapies, radiation therapy coordination, oncology follow-up, survivorship services, and supportive care. The Cancer Center Patient Navigator responsibilities include but not limited to helping patients overcome the obstacles that they may encounter while being a patient of the Cancer Center, spending time communicating with patients and their families, and guiding the patient through the cancer care continuum. The Patient Navigator collaborates closely with physicians, advanced practice providers, pharmacy, radiation oncology, laboratory, ancillary departments and community resources to ensure efficient, high-quality service delivery. May be required to work extended hours, including weekends and holidays. Support Patients in accessing healthcare and navigating health systems. Identify obstacles to care and collaborate with patients and caregivers to find viable solutions. Coordinate appointment scheduling with various healthcare providers as needed, refer patients to and collaborate with other private or public agencies, including medical service providers and support programs, as appropriate. Effectively communicate the purposes and available services to patients, their families, and caregivers. Guide patients in self-management and problem-solving, empowering them and their families to achieve optimal wellness and independence; Assist patients in securing the most beneficial and cost-effective healthcare to improve their health, safety, productivity, and overall quality of life. Promotes teamwork, accountability, and a culture of patient-centered care. Maintain up-to-date records that reflect patient progress while receiving Patient Navigator services. Prepare and keep accurate and comprehensive patient documentation. Generate related reports as needed. Ensure thorough and accurate documentation of patient interactions as required by state, county, or grantor regulations and standards.

THE MINIMUM QUALIFICATION REQUIREMENTS ARE

Applicants must meet all of the following requirements. Please note that unless specifically indicated; the required education and experiences may not be gained concurrently. In addition, qualifying work experiences are based on a 40-hour work week.

EDUCATION

Graduation from an accredited four (4) year college or university with a Bachelor's degree

EXPERIENCE

Specialized Experience:

1 ½ (1.5) years of work experience working one-on-one assisting patients with their healthcare needs in a clinic, hospital, or allied medical care facility, which involved the performance of navigating a patient through the healthcare system and demonstrated working knowledge of electronic health records system, understanding of Health Insurance Portability Accountability Act (HIPAA) regulations and familiarity dealing with various hospital staff and agencies

DESIRED SKILLS

Ability to communicate clearly and work collaboratively with physicians, staff, patients, families, and community partners. Demonstrated ability to manage multiple priorities in a fast-paced clinical environment. Proficiency in electronic medical record systems and basic computer applications. Ability to collect and use information to make sound clinical and operational decisions within the scope of responsibility. Experience with electronic medical record systems and quality improvement initiatives is desirable

Substitutions Allowed:

1. Professional patient navigator experience may be substituted for education on a year-for-year basis. 2. Possession of a bachelor's degree from an accredited college or university in a health field which included course work in anatomy, physiology and/or medical terminology courses may be substituted for six (6) months of experience. 3. Possession of a Master's degree in health care, hospital administration, public health administration, social work or nursing may substitute for one (1) year of experience.
