This is an Hourly position. Overview This job is located in Loves Park,IL Monday-Friday 8:00-5:00 Travel within service area
POSITION SUMMARY
Supports the Hospice patient, family and Hospice referral sources for the region. Acts as the Hospice Liaison for the region. Makes on site visits to referral sources and to homes of identified hospice referrals to educate them on hospice care and facilitate admission into the hospice program. Assists in completing admissions into the program by discussing hospice services and obtaining consents from the patient/family and ordering needed supplies and equipment. Prioritize and respond appropriately to various issues identified from the referral sources and/or patient and families by recommending and or seeking effective resolutions, conferring with providers as needed and collaborating with the Regional Hospice Leadership. Maintains the volunteer program for the region by recruiting, coordinating, educating, and directing volunteer services for Hospice patients and families. As part of the interdisciplinary team works closely with other members of the hospice team in assisting with holistic care of patients and families to provide support through use of volunteers. REQUIRED QUALIFICATIONS
Education:
Bachelors Other skills/knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented.
PREFERRED QUALIFICATIONS
​ Bachelor's Degree, preferred in a healthcare related field, counseling, business administration
