Job Summary We are seeking a dedicated and dynamic Community Outreach Specialist to serve as a vital link between our organization and the diverse communities we aim to support. The ideal candidate will serve as a primary liaison between the agency and the community by promoting the organization's services, building referral relationships, and coordinating exceptional client care. This role is responsible for generating new business through community outreach and marketing while ensuring clients receive outstanding service from admission through ongoing care. The ideal candidate is personable, organized, and passionate about helping individuals receive quality in-home care services. Community Outreach & Marketing Develop and maintain relationships with hospitals, physician offices, rehabilitation centers, assisted living facilities, senior centers, social workers, case managers, and other referral sources. Represent the agency at networking events, health fairs, community meetings, and educational presentations. Promote agency services to prospective clients, referral partners, and community organizations. Develop marketing strategies to increase community awareness and generate new referrals. Deliver marketing materials and maintain regular contact with referral sources. Identify opportunities to expand the agency's presence within the community. Track outreach efforts and referral activity to measure effectiveness. Client Care Coordination Meet with prospective clients and families to explain agency services and answer questions. Coordinate the client intake and admission process. Complete required documentation and obtain all necessary signatures. Collaborate with clinical and scheduling staff to ensure timely initiation of services. Conduct client welcome calls and ongoing satisfaction follow-up. Perform routine client check-ins by phone or in person to assess satisfaction and identify concerns. Address client concerns promptly and coordinate resolutions with agency staff. Assist in developing and maintaining individualized plans of care in collaboration with the care team. Maintain accurate documentation of all client communications and follow-up activities. Relationship Management Build long-term relationships with clients, families, referral partners, and community stakeholders. Maintain regular communication with referral sources regarding client admissions and service updates as appropriate. Identify opportunities to improve client satisfaction and retention. Serve as a positive ambassador for the agency at all times. Qualifications Associate's or Bachelor's degree in Healthcare Administration, Marketing, Business, Social Work, Communications, or a related field preferred. Minimum of 2 years of experience in healthcare, home care, community outreach, marketing, sales, client services, or care coordination preferred. Excellent interpersonal and relationship-building skills. Strong verbal and written communication abilities. Outstanding organizational and time management skills. Ability to multitask and prioritize in a fast-paced environment. Proficient with Microsoft programs and electronic health record (EHR) systems. Valid driver's license, reliable transportation, and proof of insurance. Preferred Skills Knowledge of home care regulations and community resources. Experience with healthcare marketing or referral development. Customer service and conflict resolution skills. Ability to work independently while collaborating with a multidisciplinary team. Public speaking and presentation experience. Physical Requirements Ability to travel locally for community outreach and client visits. Ability to sit, stand, walk, and drive for extended periods. Ability to occasionally lift up to 25 pounds.
