We partner closely with healthcare providers to ensure patients receive compassionate, whole-person care that empowers them to get back to living. About the Role We're looking for an exceptional Clinical Care Navigator to become the face of Lin Health within one of our partner specialty clinics. You'll spend your day having meaningful conversations with patients who have been living with chronic pain—helping them understand their options, answering questions with empathy, easing concerns, and guiding them through their first steps into the Lin Health program. You'll serve as a trusted extension of the clinic team, ensuring every eligible patient has the opportunity to learn about a treatment approach that could meaningfully improve their life. We're looking for someone who genuinely loves working with people, remains calm under pressure, and can build trust within minutes. This role is full- time, Monday-Friday, during standard clinic hours. What You'll Do Approximately 75%: Patient Care & Enrollment Meet one-on-one with patients referred by their healthcare providers Educate patients about the Pain Recovery Program using clear, relatable, and compassionate language Answer questions with empathy, curiosity, and professionalism Help patients determine whether the program is a good fit for their needs Guide patients through enrollment and assist them with downloading and navigating the Lin Health app Ensure every patient feels informed, supported, and confident before leaving the clinic Document patient interactions accurately and maintain appropriate records Approximately 25%: Clinic Partnership & Workflow Improvement Build strong working relationships with physicians, APPs, nurses, medical assistants, and clinic staff Collaborate with clinic leadership and the Lin Health team to identify opportunities to improve workflows and the patient experience Help optimize referral processes, patient engagement, and clinic efficiency Share feedback and insights from the clinic to help refine and strengthen the program Occasionally participate in outreach and relationship-building efforts with existing and prospective clinic partners Who You Are Required Qualifications 2+ years of clinical, patient-facing healthcare experience Experience educating patients and discussing health-related topics Excellent communication, interpersonal, and active listening skills Strong organizational skills with attention to detail Comfortable using technology, mobile applications, and electronic health records Ability to build rapport and communicate effectively with patients from diverse backgrounds Preferred Background Candidates from a variety of clinical backgrounds are encouraged to apply, including: Registered Nurse (RN) Licensed Practical Nurse (LPN) Physical Therapist (PT) or Physical Therapist Assistant (PTA) Occupational Therapist (OT) or Occupational Therapy Assistant (OTA) Medical Assistant (MA) Athletic Trainer (AT) Social Worker National Board Certified Health and Wellness Coach (NBC-HWC) Preferred Qualifications Bachelor's degree or higher Spanish language proficiency Experience with chronic pain, rehabilitation, behavioral health, or pain neuroscience Experience building relationships with healthcare providers or supporting clinic operations Demonstrated ability to identify and improve clinical workflows or patient experience What You'll Love About Lin Health Join a mission-driven healthcare company changing the future of chronic pain care Make an immediate, measurable impact on patients' lives Work alongside innovative physicians and healthcare teams Be part of a collaborative, supportive, and rapidly growing organization Opportunity for professional growth as Lin Health continues to expand
