What minimum requirements you'll need
Education:
+ High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required What additional preferences we're seeking + Strong understanding of community and public resources available to patients who are on Medicaid or uninsured. + Must be able to engage patients, build trust quickly, and assess their level of motivation and need. + Demonstrate sensitivity to diverse populations and an understanding of unique cultural barriers to care. + Excellent communication skills and the ability to refer patients to appropriate Case Manager To view full details and how to apply, please login or create a Job Seeker account
