Total Quality Management Schedule:
Full-time, Monday-Friday, 8am-5:00pm How you'll make an impact in this role: Bridges care gaps: Connects pregnant patients to prenatal visits, mental health support, and social services to reduce missed appointments.
Empowers families:
Educates patients on pregnancy milestones, warning signs, and self-advocacy to build confidence.
Reduces stress:
Helps families navigate insurance, transportation, and medical paperwork so they can focus on health.
Builds trust:
Services as a consistent, supportive point of contact between the patient and the clinical care team What minimum qualifications you'll need:
Education:
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required What additional requirements you'll need: Associates degree in Social Work or Psychology preferred Familiarity navigating the Medicaid system Familiar with community resources Life at
