Patient Care Navigator Are you passionate about helping others and making a difference in the healthcare journey? We are seeking a compassionate, organized, and customer-focused Patient Care Navigator to join our team. In this role, you'll help guide patients and their families through the admissions process while ensuring a smooth, positive experience from referral to discharge. What You'll Do Serve as the primary point of contact for patients, families, referral sources, and healthcare providers. Coordinate the admission process and assist with transitions of care. Conduct facility tours and educate patients and families on available services. Build and maintain relationships with hospitals, physicians, and community referral partners. Verify insurance information and assist with admission paperwork. Collaborate with nursing, therapy, social services, and other departments to ensure seamless patient care. Maintain accurate referral and admission records. Follow up with referral sources and prospective patients in a timely manner. Deliver exceptional customer service while advocating for patients and their families. Qualifications Experience in healthcare admissions, case management, customer service, marketing, or care coordination preferred. Excellent communication and interpersonal skills. Strong organizational and multitasking abilities. Proficiency with Microsoft Office and electronic medical record systems. Ability to work independently and as part of a collaborative team. Long-term care or skilled nursing experience is a plus.
