Role:
As a Palliative Care Nurse Navigator, you'll help support patients and families throughout their healthcare journey. Responsibilities include coordinating care between inpatient and outpatient settings, participating in hospital rounds, conducting patient follow-up, facilitating transitions to hospice services when appropriate, providing education to patients and families, and serving as a resource for providers and clinical staff. This role combines the relationship-centered aspects of traditional nurse navigation with the unique opportunity to support patients across multiple care settings, ensuring continuity, communication, and compassionate care throughout the treatment process. Position Summary:
Serves as a patient navigator throughout the continuum of care.
Education, License & Cert:
Diploma from an accredited school/college of nursing Required RN licensure at time of hire.
Essential Functions:
Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.| Coordinates efforts for treatment and promotes quality improvement and educational efforts. Compiles data, tracks outcomes, and makes recommendations for process improvement. Provides referrals/support for follow-up care for patients who are uninsured or underinsured.
