Role:
The Chronic Care Management position is essential in providing comprehensive support to patients with chronic conditions, ensuring they receive the necessary care and resources to manage their health effectively. This role involves coordinating care plans, facilitating communication between patients and healthcare providers, and monitoring patient progress to improve health outcomes. Minimum Qualifications:
Certification in Medical Assisting or equivalent healthcare qualification. Proficiency in bilingual communication, with the ability to converse fluently in English and another language.
Preferred Qualifications:
Experience in chronic care management or a related healthcare field. Familiarity with electronic health records (EHR) and care management software.
Bilingual Responsibilities:
Develop and implement individualized care plans for patients with chronic conditions. Coordinate communication between patients, healthcare providers, and specialists to ensure seamless care.
