Job Description

Pay:
$24.00-$27.00 per hour (DOE/DOQ)
Schedule:
Full-Time, 40 hours/week,

Care Navigation Supervisor Benefits Medical, Dental & Vision Insurance Paid Vacation, Sick Leave, Float Days & Holidays 403(b) Retirement Plan Life Insurance & Long-Term Disability Employee Assistance & Wellness Programs LifeFlight Membership Education Allowance Position Summary We're seeking compassionate and organized Care Navigators to support individuals with behavioral health needs by coordinating services, connecting clients to community resources, and promoting treatment engagement and recovery. This role works closely with clinical teams, healthcare providers, and community partners to ensure continuity of care and positive client outcomes. Key Responsibilities Coordinate care across behavioral health, medical, social service, housing, education, and employment providers. Support treatment engagement, retention, and follow-up care. Develop and maintain individualized service and crisis plans. Assist clients with goal setting, skill development, symptom management, and accessing community resources. Coordinate transitions from hospitals, treatment facilities, and other care settings back into the community. Document services accurately and maintain confidentiality standards. Participate in team meetings and collaborate with multidisciplinary providers.
Qualifications Required:
Bachelor's degree in Social Services, Psychology, Human Services, or a related field. Two years of experience working with individuals experiencing emotional, behavioral, or mental health challenges. Valid driver's license with an insurable driving record. Ability to obtain Agency Affiliated Counselor Registration (assistance provided).
Additional Requirements:
Pre-employment drug screen and background check. Proof of immunizations (MMR, Hep B, Tdap, and current flu vaccine). Preferred Skills Strong care coordination and case management abilities. Knowledge of behavioral health recovery principles and community resources. Excellent communication, organization, and problem-solving skills. Ability to work independently while collaborating with a multidisciplinary team.
