Community ambulatory Care Pharmacist A Family Pharmacy - 4.4 Hesperia, CA Job Details Part-time $60 - $80 an hour 18 hours ago Qualifications Customer communication Full Job Description Job Overview We're looking for a Community Ambulatory Care Pharmacist to join our team in a hybrid position that bridges traditional retail pharmacy with advanced ambulatory care services. This role is ideal for a pharmacist who wants to practice at the top of their license — combining day-to-day dispensing with direct patient care, clinical consultations, and preventive health services. Duties Provide patient consultations on medications, disease states, and treatment plans Administer vaccines (flu, COVID-19, shingles, and routine immunizations) Support smoking cessation through nicotine replacement therapy counseling Furnish naloxone and provide overdose prevention education Conduct HIV PrEP/PEP consultations and furnish medications per protocol Establish and manage diabetes care protocols, including point-of-care testing and medication therapy management Collaborate with prescribers and care teams to optimize patient outcomes Maintain accurate documentation of clinical services provided Balance clinical service delivery with core retail pharmacy operations (dispensing, verification, patient safety checks) Experience Prior experience in community pharmacy practice with a focus on patient service delivery and clinical pharmacy functions. Demonstrated competence in managing pharmacy teams or leading pharmacy operations within ambulatory or LTC settings. Retail experience or residency training that enhances skills in complex medication management and medical terminology understanding. Inventory control, and regulatory compliance procedures. Knowledge of physiology, anatomy, pharmacology principles related to pediatrics, pain management, and general health conditions. Certification in immunizations, basic life support (BLS), Strong communication skills for effective client interaction and collaboration with multidisciplinary healthcare providers while maintaining clinical confidentiality policies.
