Category Pharmacy - Pharmacist Job Type Contract Join a prominent healthcare system as a Pharmacist specializing in Transitions of Care/Ambulatory Care. In this key role, you will work closely with patients before hospital discharge to provide essential medication education and counseling, ensuring they understand their medication regimens. By assisting with medication refills prior to discharge, you help improve patient compliance and promote better health outcomes—supporting a "meds to beds" model designed to reduce readmissions. This opportunity is ideal for new PGY1 residents, though experienced non-PGY1 pharmacists, including those from retail or outpatient settings, are also encouraged to apply. Fluency in Spanish is a valuable asset and will enhance the impact you make in this diverse patient population.

Whittier, CA Responsibilities:
Collaborate with interdisciplinary teams to support patient discharge planning through medication reconciliation and education Counsel patients and caregivers on proper medication use, side effects, and adherence strategies Facilitate medication refills to ensure patients leave with necessary prescriptions Document interventions and communicate with healthcare providers to optimize medication therapy Contribute to the success of the "meds to beds" program, improving transition of care outcomes
Desired Experience & Qualifications:
Licensed Pharmacist with active license in good standing Preferably new PGY1 residents, but open to retail/outpatient pharmacists looking to transition Strong patient counseling and communication skills Spanish language proficiency is a plus Ability to work effectively in a fast-paced hospital environment Commitment to enhancing patient care and compliance during transitions
Additional Information:
This contract role is part of a contract-to-hire track within a large county health system known for its comprehensive ambulatory and inpatient services. Starting on contract allows for a timely onboarding process ahead of permanent placement, which can take several months.
