Job Description

Delta Health is seeking a Clinical Pharmacist to provide safe, high-quality inpatient and outpatient pharmacy services. The Pharmacist is responsible for medication preparation and dispensing, medication management, clinical support, and compliance with hospital, state, and federal regulations. Key Responsibilities Review and interpret medication orders and identify potential therapeutic issues. Prepare, compound, package, label, and dispense sterile and non-sterile medications. Supervise inpatient and outpatient medication preparation and dispensing. Monitor drug therapies and provide clinical recommendations and interventions. Perform medication reconciliation and discharge medication counseling. Conduct medication therapy management under approved collaborative practice agreements. Provide drug information and education to physicians, nurses, patients, and other healthcare professionals. Oversee pharmacy technician workflow and verify medication preparation, labeling, order entry, and charges. Maintain compliance with Colorado Board of Pharmacy, DEA, FDA, and hospital requirements, including controlled substance records and medication inspections. Participate in quality improvement, auditing, education, and other pharmacy initiatives. Maintain a safe, clean, and compliant pharmacy environment. Share on-call responsibilities with other pharmacists. Qualifications Bachelor of Science in Pharmacy (B.S. Pharmacy) required. Current Colorado Pharmacist (RPh) license required. Previous hospital pharmacy experience preferred. Hospital pharmacy residency preferred. Experience with unit-dose, IV admixture, TPN, and/or pharmacokinetics preferred.
