Job Description

Overview We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for our patients. This position requires strong clinical skills, attention to detail, and a commitment to providing exceptional patient care in a hospital or clinical setting. As a Pharmacist, you will collaborate with healthcare professionals to optimize medication therapy and improve patient outcomes. Responsibilities Review and interpret physician orders to ensure appropriate medication therapy. Prepare and dispense medications accurately while adhering to aseptic techniques. Provide medication administration support and counsel patients on proper medication usage. Monitor patient responses to medications, making adjustments as necessary in collaboration with the healthcare team. Conduct medication therapy management and provide recommendations for medical management. Educate patients and caregivers on drug interactions, side effects, and proper administration techniques. Maintain accurate records of prescriptions, patient profiles, and medication inventory. Participate in clinical rounds, particularly in acute care settings or pediatrics as needed. Supervise pharmacy technicians and support staff in daily operations. Qualifications Doctor of Pharmacy (Pharm.

D.) degree from an accredited institution. Active pharmacist license in the state of practice. Experience in hospital pharmacy or completion of a pharmacy residency is preferred. Strong knowledge of physiology and pharmacology as it relates to patient care. Proficiency in medication administration practices and aseptic techniques. Excellent communication skills with the ability to work collaboratively within a multidisciplinary team. Previous experience working with pediatric patients is a plus.
