Tallahassee, FL 💼 Position Overview As a Clinical MTM Pharmacist, you will play a critical role in improving patient health outcomes through: ✅ Conducting outbound patient calls (approximately 100 calls daily) ✅ Performing Comprehensive Medication Reviews (CMRs) ✅ Delivering in-depth consultations regarding statin medication use following AHA guidelines ✅ Providing Medication Therapy Management (MTM) services ✅ Reviewing patient medications and identifying opportunities for optimization ✅ Accurately documenting prescription, diagnosis, and recommendation data into a web-based application ✅ Collaborating with the MTM team to support quality and patient care initiatives 🎯 What We're Looking For ✔️ Active Florida Pharmacist License (Please include a screenshot of your Florida Pharmacist License at the top of your resume.) ✔️ Minimum 4 years of Clinical MTM Pharmacist experience ✔️ Experience conducting medication reviews ✔️ Strong understanding of pharmaceutical and medical terminology ✔️ Familiarity with Medicare Part D Star Measures, including:

• SUPD
• SPC
• CMR ✔️ Exceptional verbal and written communication skills ✔️ Fluently speak, read, and write English ✔️ Strong computer and technology skills ⭐ Preferred Qualifications ✅ Management or leadership experience ✅ Salesforce experience ✅ Previous call-based MTM experience ✅ Experience working in quality improvement initiatives 📋 Responsibilities
