Category Pharmacy - Pharmacist Job Type Contract An experienced pharmacist is needed to provide comprehensive pharmacy services at a 35-bed long-term acute care hospital (LTACH) specializing in critical illness recovery. This role emphasizes clinical expertise, medication management, and regulatory compliance within an inpatient setting.

Contract Key Responsibilities:
Perform thorough medication reconciliation and develop accurate prior-to-admission medication lists using multiple data sources. Provide discharge medication counseling to patients and their families. Monitor drug therapy for contraindications, interactions, allergies, and appropriate dosing. Offer clinical consultations and make medication recommendations to healthcare providers. Participate actively in antimicrobial stewardship programs including interpreting culture results and advising on treatment for infections such as UTI, pneumonia, and SSTI. Conduct pharmacokinetic and anticoagulation dosing and monitoring. Support medication-use review, quality assurance, and Pharmacy & Therapeutics committee activities. Manage medication inventory, preparation, sterile compounding, labeling, and dispensing according to safety standards. Ensure compliance with federal and state pharmacy laws, JCAHO requirements, and facility policies. Participate in staff training and provide education to healthcare team members and patients.
Ideal Qualifications:
Active pharmacist license applicable to the assignment. Hospital or inpatient pharmacy experience preferred. Proficiency with EPIC electronic health records system. Strong clinical and communication skills. Ability to work independently in a fast-paced LTACH environment. Experience in medication reconciliation, antimicrobial stewardship, pharmacokinetics, and anticoagulation management.
