Job Description

Overview Join our dynamic healthcare team as a Specialty Infusion Pharmacist, where your expertise will directly impact patient outcomes through specialized medication management and infusion therapy. This role offers an exciting opportunity to work within a fast-paced, multidisciplinary environment, ensuring safe and effective delivery of complex infusions across diverse patient populations. As a vital member of our pharmacy team, you will utilize your advanced clinical knowledge to optimize pharmacotherapy, ensure regulatory compliance, and enhance patient care experiences. Responsibilities Prepare and dispense sterile and non-sterile infusion medications with precision, adhering to aseptic techniques and pharmaceutical regulations. Collaborate closely with healthcare providers to develop individualized infusion plans, including oncology, GI , pediatrics, neurology and immunotherapy . Conduct comprehensive patient assessments to evaluate medication appropriateness, potential interactions.. Monitor patient responses during infusion therapy, adjusting medications as needed while ensuring safety and efficacy. Maintain accurate documentation within Electronic Medical Records (EMR/EHR) systems, including compound 360, Pioneer and salesforce. Ensuring compliance with pharmaceutical regulatory standards with ACHC and PCAB accreditation . Provide expert consultation on pharmacology, drug regulation, and medication administration best practices to clinical staff and trainees. Requirements Valid pharmacist license in IL and MI Extensive clinical pharmacy experience in specialty home infusion setting Demonstrated knowledge of pharmacotherapeutics, pharmacology, medical terminology, anatomy, physiology, and drug regulation standards. Hands-on experience with IV infusion preparation, aseptic technique. Sterile and non-sterile compounding experience is a plus. Proficiency with Pharmacy Information Systems (PIS) and EMR/EHR platforms; Experience with Pioneer, Salesforce and compounding 360 is recommended Ability to perform patient assessment accurately while managing complex medication regimens for diverse populations including pediatrics and patients with Chronic illness. Excellent communication skills for patient service delivery and interdisciplinary collaboration; experience in precepting or mentoring is a plus. Pay:
$135,000.00 - $155,000.00 per year
