OVERVIEW
Infinium Pharmacy is a privately owned and independently operated, Long Term Care Pharmacy, dedicated to meeting the growing need for personalized pharmaceutical care in Skilled and Assisted Living facilities.
PURPOSE OF POSITION
The Clinical Pharmacist is essential to ensure accurate and complete service for patients. This position reviews profiles for correct drug, dosage, and route of administration; ensures that no drug duplication or drug-drug interaction exists; ensures that patient is not allergic to any medication prescribed. This position may also require medication therapy management training and implementation as well as monthly medication reviews for the facilities, as required. The Clinical Pharmacist may fulfill the role of a Consultant Pharmacist.
SUMMARY OF DUTIES
Reviews prescription orders issued by physicians and other authorized prescribers Performs medication profile reviews Examines prepared orders for accuracy Provides clinical drug information to meet the needs of the patient, physician, or other healthcare professional Coordinates the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies Provide quality on-call pharmacist services during closed hours of pharmacy Maintains established procedures concerning medication packaging standards, quality assurance, and security of controlled substances Ensures compliance with federal, state, and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records Assist with any duties necessary to ensure the functionality of the business Participates in consulting pharmacist duties Monthly Medication reviews for residents of nursing facilities, either on-site or off-site, as deemed appropriate. Medication room inspections at nursing facilities as required by regulation or by contract. Medication Pass Observations at nursing facilities as required by contract. Attendance at Facility Meetings (as contracted) and presentation of relevant clinical information. Other duties as assigned.

ABILITIES
Education Doctorate of Pharmacy degree from an accredited college or university required. Missouri Pharmacist License (must be in good standing) Qualifications Strong data entry/computer processing skills Strong organizational and time management skills Great attention to detail, accuracy, and thoroughness Ability to work in a fast-paced environment Compliance to all HIPAA/privacy regulations Competencies Oral Communication

Work Environment/Physical Demands Continually utilize visual acuity to read fine and/or small print, and to work off a computer screen for up to 10 hours/day Frequently sitting and/or standing for up to 10 hours/day Having a range of motion from kneeling, to bending over to raising arms above head to frequently retrieve stock Frequently required to lift and/or carry up to 40 pounds
