Home Infusion Staff Pharmacist - Irving, TX RPh on the Go - 4.1 Irving, TX Job Details Contract 1 day ago Qualifications Customer communication Licensed Pharmacist (State-Specific) Licensed Pharmacist Home health Full Job Description Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered. A Sterile Compounding Pharmacist opportunity is available in Irving, TX on a 13-week contract. This position requires an immediate start and offers a schedule from 7:00 AM to 3:30 PM.
Responsibilities:
Evaluate patients for eligibility for home infusion therapy and home-based care. Develop, monitor, and adjust individualized medication therapy plans utilizing clinical assessments, laboratory values, and pharmacokinetic principles. Prepare, verify, and dispense sterile medications in strict compliance with USP standards, state and federal regulations, and pharmacy policies. Educate patients, caregivers, and healthcare providers on medication therapy, infusion administration, and proper medication usage. Collaborate with healthcare providers to ensure continuity of care and monitor patient outcomes. Manage infusion pumps and medication administration equipment, upholding quality assurance and safety standards. Provide drug information, maintain accurate documentation, and support regulatory compliance and medication safety initiatives.
Qualifications:
Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy. Active, unrestricted Pharmacist license in Texas. One or more of the following: Minimum one year of sterile IV compounding experience. Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM). Completion of an ASHP-accredited Pharmacy Residency. Completion of an Advanced Pharmacy Practice Experience (APPE). Equivalent pharmacy experience. Strong clinical knowledge of home infusion pharmacy practices. Ability to interpret laboratory values and apply pharmacokinetic principles. Excellent communication, patient counseling, and customer service skills. This role offers a challenging environment for pharmacists skilled in sterile compounding and home infusion therapy.
