Responsibilities Verify CPOE prescription medications accurately while adhering to all state and federal pharmacy laws and regulations. Conduct comprehensive patient assessments to determine appropriate medication therapies and identify potential drug interactions. Prepare and compound medications, including sterile preparations using aseptic techniques in accordance with USP 797 standards. Manage pharmacy operations efficiently, including inventory control, medication ordering, and ensuring proper storage conditions. Collaborate with healthcare providers to develop and implement effective medication management plans for diverse patient populations, including pediatrics and long-term care residents. Maintain strict confidentiality of patient information following HIPAA guidelines and clinical confidentiality policies. Utilize pharmacy software systems (PIS) and electronic health records (EHR) to document patient interactions accurately and streamline workflow. Ensure compliance with pharmaceutical regulatory requirements, including drug regulation policies and pharmacy practice standards. Support clinical initiatives related to pain management, acute care, and medical management in various healthcare settings such as hospitals or community pharmacies. Skills Proven experience in hospital pharmacy practice with a focus on acute care hospital setting. Strong knowledge of clinical pharmacy in managing kinetic dosing for vancomycin and other antibiotics. Demonstrated ability to practice as a licensed pharmacist with current state registration; additional certifications such as Basic Life Support (BLS) are preferred. Expertise in managing pharmacy teams or supervising pharmacy technicians to ensure smooth operations. Familiarity with pharmacy information systems (PIS), EMR/EHR systems, and inventory management tools for efficient workflow management. Excellent customer service skills with the ability to communicate effectively with patients and healthcare providers about medication use and health concerns. Experience in compounding medications using aseptic techniques while maintaining sterile environment standards such as USP 797 guidelines.
