This role includes evaluating patients, developing individualized treatment plans, improving functional mobility, educating patients and caregivers, coordinating care with the interdisciplinary team, and documenting patient progress in compliance with agency policies and state regulations. What You'll Do Perform comprehensive physical therapy evaluations and home safety assessments. Develop and implement individualized treatment plans based on physician orders. Assess strength, mobility, gait, range of motion, balance, and rehabilitation potential. Provide skilled physical therapy interventions to improve function and independence. Educate patients and caregivers on exercises, mobility techniques, fall prevention, and the proper use of therapeutic equipment. Monitor patient progress and modify treatment plans as appropriate. Communicate changes in patient condition to the physician and interdisciplinary team. Complete accurate and timely clinical documentation and progress notes. Participate in discharge planning and case conferences. Identify the need for additional home health services and make appropriate referrals. Supervise Physical Therapist Assistants (PTAs) in accordance with agency policy and state regulations. Supervise Home Health Aides every two weeks when physical therapy is the only skilled service. QualificationsRequired Current California Physical Therapist License Degree from an accredited Physical Therapy program Current CPR/BLS Certification Valid California Driver's License Reliable transportation with current auto insurance Ability to travel throughout assigned service areas Strong communication, organization, and clinical assessment skills Ability to work independently with minimal supervision Preferred Minimum of 2 years of Physical Therapy experience Home Health or Community Health experience Experience using WellSky or other Home Health EMR systems Bilingual is a plus Required Credentials California Physical Therapist License Driver's License Auto Insurance CPR/BLS Certification TB Test or Chest X-Ray Physical Examination Professional Liability SSN Physical Requirements Ability to safely assist, lift, position, or transfer patients as needed. Ability to travel between patient homes throughout the assigned territory. Ability to lift therapy equipment and supplies. Frequent walking, standing, bending, kneeling, and reaching. Pay:
$80.00 - $110.00 per hour Application Question(s): Are you a licensed Physical Therapist (PT) in the State of California? Are you willing to travel within a 20 up to 30 mile radius (if needed) to visit patients in their homes?
