Lakeside is a five-star facility specializing in psychiatric care. Together with our nursing staff, our goal is to help patients achieve their highest level of functional independence. We provide long-term care to patients 18 years of age or older, both male and female, who display severe and persistent psychiatric impairment and whose adaptive functioning is moderately to severely impaired. These patients require 24-hour care. Due to their chronic mental illness, admission to a standard skilled nursing facility is prohibitive. We employ our own therapists, who work together with our nursing staff to help patients achieve their highest level of functional independence. We have 58 beds in our program, dedicated to improving or preventing deterioration of our residents' current level of functioning. The recovery, wellness, and discovery approach involves collaborating with the resident to facilitate hope and empowerment, with the goals of counteracting internal and external "stigma," improving self-esteem, encouraging resident self-management of his or her life and making his or her own choices, reintegrating the resident into his or her community as a contributing member, and achieving a satisfying and fulfilling life. Job Description:
The primary role of the Physical Therapist is to provide and direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to organizations guidelines, professional standards and community needs. This position will also serve as a patient and rehabilitation advocate for current prospective patients, to all internal and external customers while participating in quality assurance and customer service activities.

Pay:
$50.00 - $55.00/hr Qualifications Graduate of an approved curriculum in Physical Therapy. Is able to practice unencumbered. Is in good standing with all regulatory agencies and licensing boards. Working knowledge of Medicare and other payor sources. Full knowledge of resident's rights. Exudes professionalism in presentation. Able to work on multitasks at the same time. Must be able to read, write, speak, understand and communicate in the English language.
