In this dynamic role, you will deliver expert physical therapy care directly in patients' homes, empowering individuals to regain mobility, improve function, and enhance their quality of life. Your expertise will foster meaningful patient interactions, develop personalized treatment plans, and collaborate closely with healthcare professionals to ensure optimal recovery outcomes. Responsibilities Conduct comprehensive assessments of patients' physical conditions within their home environments, including evaluations of mobility, strength, and functional ability Develop and implement individualized care plans tailored to each patient's specific needs, including discharge planning and goal setting Deliver evidence-based physiotherapy interventions such as therapeutic exercises, manual therapy, trigger point therapy, and gait training to promote recovery and independence Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare providers to coordinate patient care and adjust treatment strategies as needed Maintain accurate clinical documentation adhering to standards for medical terminology, physiotherapy practices, and electronic health records (EHR) management systems Educate patients and their families on treatment techniques, safety precautions, and home exercise programs to support ongoing rehabilitation Monitor patient progress regularly, adjusting treatment plans to optimize outcomes and facilitate safe discharge from services Requirements Valid state licensure as a licensed Physical Therapist with practicing credentials in physical therapy Proven experience in home health settings or outpatient clinics with a focus on geriatrics, orthopedics, neurology or pediatrics is highly preferred Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards Hands-on experience with outpatient rehabilitation centers or inpatient hospital environments is advantageous Demonstrated ability to develop effective treatment plans for diverse patient populations including geriatric patients and those recovering from orthopedic or neurological conditions Familiarity with EMR systems or electronic health records (EHR) management for accurate documentation of patient care activities Excellent client communication skills with the ability to educate patients and collaborate seamlessly with healthcare teams Working knowledge of home & community care practices along with outpatient clinic experience is desirable Join us in delivering compassionate, expert physical therapy that transforms lives!
