Position Overview Evergreen Physical Therapy Services is seeking a dedicated Full-Time Physical Therapist (PT) to join our dynamic team at our state-of-the-art Rehabilitation Center located in Bethel, Connecticut. Work Environment The Physical Therapist will work onsite at our Bethel Rehabilitation Center, which is equipped with advanced therapy technologies and a warm, welcoming atmosphere. Key Responsibilities Conduct comprehensive evaluations, including the review of patient history, performance of specific assessments, and documentation of results. Articulate evaluation findings and collaboratively design treatment plans with patients and their families, setting clear and achievable goals. Implement individualized treatment protocols based on evaluation outcomes and ongoing patient assessments, ensuring adherence to best practices in physical therapy. Collaborate with a multidisciplinary team, consulting with fellow healthcare professionals when needed, and adapting plans of care based on changes in patients' conditions. Oversee patient discharge planning and follow-up care, ensuring that all documentation accurately reflects the course of care, including initial evaluations and progress notes. Manage time effectively to meet productivity standards while maintaining a flexible schedule for patients and team meetings. Contribute to departmental objectives by actively engaging in quality improvement initiatives and complying with regulatory standards. Pursue continuing education for professional growth, serving as a mentor and clinical instructor to physical therapy students when applicable. Enhance customer satisfaction by fostering positive patient interactions and addressing any access issues to care. Patient Population / Client Focus The rehabilitation services at Evergreen cater to a diverse patient population, ranging from post-operative recovery patients to individuals managing chronic pain conditions. Our focus is on creating personalized therapy plans that not only address physical ailments but also empower patients through education and active participation in their recovery journey. Qualifications A Bachelor's degree in Physical Therapy is required; a Doctorate degree in Physical Therapy (DPT) is preferred. Active state license in physical therapy is mandatory. Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is required. Demonstrated commitment to professional development and advancement in the field of physical therapy. Preferred Experience A minimum of 2 years of relevant experience in an outpatient physical therapy setting is preferred but not mandatory. Prior experience with various therapeutic technologies and methodologies advantageous. Compensation and Benefits The annual salary for this position ranges from $100,000 to $120,000, commensurate with experience.
