Position Summary The Physical Therapist will provide skilled physical therapy services to patients in their homes, helping them improve mobility, strength, balance, independence, and overall quality of life. Responsibilities Perform comprehensive physical therapy evaluations and assessments. Develop and implement individualized treatment plans. Provide skilled therapeutic exercises, mobility training, gait training, and other appropriate interventions. Educate patients and caregivers regarding exercises, safety, and home programs. Monitor patient progress and modify treatment plans as needed. Complete all required clinical documentation accurately and timely. Communicate with physicians, nurses, caregivers, and other members of the interdisciplinary team. Follow Medicare, Medicaid, AHCA, and agency requirements and standards. Qualifications Current and unrestricted Florida Physical Therapist (PT) license . Graduate of an accredited Physical Therapy program. Current CPR certification. Previous home health experience preferred, but not required. Strong communication, organization, and clinical documentation skills. Reliable transportation and ability to travel to patients' homes.
