Duties Conduct comprehensive evaluations of patients' physical conditions, including mobility, strength, and functional abilities. Develop and execute tailored treatment plans utilizing evidence-based physical therapy techniques such as trigger point therapy, orthopedics, neurology interventions, and geriatrics care. Manage discharge planning processes to ensure seamless transition of patients back into the community or home environment. Collaborate closely with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to coordinate patient care strategies. Document all clinical assessments, treatment sessions, progress notes, and discharge summaries in accordance with clinical documentation standards and electronic health records (EHR/EMR systems). Provide patient education on injury prevention, rehabilitation exercises, and health maintenance strategies to empower self-care. Participate in outpatient rehabilitation programs at clinics or outpatient centers specializing in pediatrics, geriatrics, orthopedics, or neurology cases. Experience Proven experience practicing in a physical therapy role within inpatient hospital settings, outpatient clinics, or home health environments. Demonstrated expertise in acute care scenarios with knowledge of physiology and medical terminology relevant to complex patient conditions. Hands-on experience developing treatment plans for diverse populations including geriatric patients, pediatric clients, and those undergoing orthopedic or neurological rehabilitation. Familiarity with clinical documentation standards and proficiency managing electronic health records (EHR/EMR systems). We use WebPT. Strong communication skills to effectively engage with patients and collaborate with multidisciplinary healthcare teams. Prior work involving discharge planning and working within outpatient rehabilitation centers or inpatient hospital units is highly desirable. Knowledge of physiotherapy techniques such as trigger point therapy and an understanding of anatomy to tailor interventions effectively.
