Develop and execute tailored care plans that incorporate evidence-based physiotherapy techniques such as trigger point therapy and orthopedics interventions. Deliver hands-on therapy sessions focusing on improving movement, alleviating pain, and restoring function in outpatient, inpatient, or home & community care settings. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions between care environments. Monitor patient progress regularly, adjusting treatment plans as needed to optimize outcomes across acute care, hospital experience, and home health contexts. Educate patients and their families on injury prevention, exercises, and self-care techniques to promote long-term wellness. Maintain accurate documentation within EMR (Electronic Medical Records) systems while adhering to healthcare regulations and standards. Qualifications Bachelor's or Master's degree in Physiotherapy or an equivalent credential recognized for practice in your region. Valid licensure or registration as a licensed Physical Therapist. Proven experience working in inpatient hospital settings, outpatient clinics, or home health environments with a focus on patient care excellence. Strong knowledge of anatomy and physiology principles relevant to orthopedics, geriatrics, pediatrics, and general rehabilitation. Familiarity with EMR systems for documentation and care coordination purposes. Experience in discharge planning, acute care procedures, and developing comprehensive care plans tailored to individual needs. Excellent communication skills with the ability to educate patients effectively on physiotherapy techniques. Nice-to-have skills include experience with home & community care programs, outpatient therapy services, trigger point therapy techniques, and familiarity with medical terminology used in hospital environments.
