Work Environment The Physical Therapist will primarily work onsite in patients' homes across Palm Coast, Florida, allowing for an intimate approach to rehabilitation that acknowledges each patient's unique circumstances. You will typically work a standard full-time schedule, ensuring consistent and quality care. Key Responsibilities Conduct thorough assessments of patients' physical conditions, utilizing observational skills and tests to evaluate mobility, strength, and functional limitations. Develop and implement individualized treatment plans that align with best practices for orthopedic rehabilitation, setting specific, measurable goals for each patient. Change therapy plans as necessary based on patient progress, feedback, and data, ensuring a flexible approach that can adapt to patient needs over time. Educate patients and their families on therapeutic exercises, lifestyle adjustments, and measures to complement treatment, fostering a comprehensive understanding of their recovery process. Collaborate with physicians and interdisciplinary teams to communicate patient status, contribute to care discussions, and help refine treatment approaches based on collective insights. Maintain accurate and timely documentation for all patient interactions, ensuring all records reflect the standards set forth by state regulations and organization policies. Educate patients on the use of therapeutic equipment, observing all safety precautions and ensuring they understand proper techniques for continued progress at home. Support and mentor junior therapists or interns, contributing to the overall development of the physical therapy team. Patient Population / Client Focus The successful candidate will engage with a diverse clientele that often includes elderly patients recovering from joint replacements, younger athletes rehabilitating from sports injuries, and individuals with chronic orthopedic conditions. Your role will be crucial in developing tailored rehabilitation plans that consider the unique needs of each patient, contributing significantly to their physical recovery and emotional outcomes. Qualifications Bachelor's degree or higher from an accredited program in physical therapy. Currently licensed as a Physical Therapist in the state of Florida; temporary permits will also be considered. Certification in Basic Life Support (BLS) for Healthcare Providers. Strong analytical and communication skills, with an emphasis on empathy and patient-centered care. Preferred Experience Prior experience in a home care setting is highly advantageous, particularly in orthopedic rehabilitation. Experience working with older adults or patients with chronic orthopedic conditions. Familiarity with electronic health record (EHR) systems and documentation practices within a healthcare setting. Compensation and Benefits This position offers a competitive salary ranging from
USD 120,000
to 130,000 annually, commensurate with experience and qualifications.
