What You'll Be DoingDeliver Exceptional Therapy Complete in-home assessments focused on mobility, balance, and strength Develop and implement individualized treatment plans Provide therapeutic interventions and exercise progression Recommend adaptive equipment and safety modifications Educate patients and caregivers to maximize outcomes Collaborate Across The Continuum Partner with nurses, aides, physicians, and interdisciplinary teams Communicate progress and adjust treatment plans as needed Document visits and monitor patient outcomes Help patients remain strong, safe, and independent Required Qualifications Active Minnesota Physical Therapist license Valid driver's license and reliable transportation Strong communication and clinical reasoning skills Ability to work independently and manage your schedule effectively Preferred Qualifications Home health experience Outpatient, inpatient, SNF, or rehabilitation experience Passion for relationship-based, patient-centered care Great Therapy Isn't Measured In Units.
