Your next patient may have Parkinson's disease, generalized weakness after hospitalization, repeated falls, or a caregiver who isn't confident assisting with transfers. You assess the patient, understand the environment, identify the barriers, and build a plan around what that person actually needs to accomplish. The question is whether what you're doing actually changes the patient's ability to function when you're gone. Can they get from the bedroom to the bathroom safely? Can they manage the steps to leave their house? Can their spouse assist with a transfer without putting either of them at risk? What equipment would make the biggest difference? What You'll Be Doing Perform comprehensive in-home Physical Therapy evaluations Develop individualized plans of care based on functional needs and goals Provide skilled interventions addressing strength, balance, gait, transfers, and mobility Assess fall risks and identify barriers within the patient's home Recommend adaptive equipment and environmental modifications Educate patients and caregivers on mobility, safety, and continued progress Monitor outcomes and modify treatment plans based on clinical findings Collaborate with physicians, nurses, PTAs, OTs, and other interdisciplinary clinicians Complete accurate and timely documentation Travel throughout Southwest Minnesota based on patient needs The PTs Who Thrive Here Like Figuring Things Out We're looking for someone who enjoys clinical independence and doesn't need every patient, environment, or day to look the same. You'll need to be organized enough to manage a field-based schedule, comfortable making decisions independently, and confident adapting your approach when the obvious solution isn't necessarily the best one. Required Qualifications Active Minnesota Physical Therapist license Valid driver's license and reliable transportation Willingness to travel throughout Southwest Minnesota Strong clinical assessment and problem-solving skills Excellent patient and caregiver communication Ability to independently manage a field-based schedule Preferred Qualifications Home health experience OASIS experience Experience with geriatric, neurological, orthopedic, or medically complex patients Strong interest in functional, one-on-one Physical Therapy Use More of What You Learned to Become a PT Your clinical training gave you the ability to assess movement, identify limitations, understand why they're happening, and determine what can be done about them.
