Non-Exempt (Hourly) Position Summary The Physical Therapist (PT) provides skilled physical therapy services under physician or qualified provider referral and is responsible for evaluating patients, developing individualized treatment plans, and implementing evidence-based interventions to restore function, improve mobility, reduce pain, prevent disability, and maximize independence and quality of life. The setting includes short-term rehabilitation, outpatient therapy and long-term care. The PT utilizes an understanding of the principles of growth and development to assess each patient's age-specific needs and provide appropriate treatment and care across the continuum of services. The PT participates in the interdisciplinary care planning process to ensure patient and resident needs are met and provides clinical supervision to Physical Therapist Assistants (PTAs) in accordance with New York State practice regulations. The Physical Therapist promotes Cuba Memorial Hospital (CMH) in a positive and professional manner throughout the organization and within the community. Essential Responsibilities Perform comprehensive physical therapy evaluations using standardized assessments and clinical judgment. Develop individualized plans of care with measurable, functional treatment goals. Provide skilled physical therapy interventions including therapeutic exercise, gait training, balance training, neuromuscular re-education, manual therapy, transfer training, therapeutic activities, and functional mobility training. Evaluate and recommend assistive devices, adaptive equipment, and mobility aids to maximize patient safety and independence. Reassess patient progress and modify treatment plans as clinically indicated. Complete timely, accurate, and compliant documentation according to facility policies, regulatory requirements, and payer guidelines. Provide clinical supervision and direction to Physical Therapist Assistants (PTAs) in accordance with New York State regulations. Educate patients, families, caregivers, and staff regarding treatment plans, home exercise programs, fall prevention strategies, mobility, and discharge recommendations. Participate in interdisciplinary care planning, family meetings, discharge planning, and quality improvement initiatives. Maintain compliance with infection prevention, patient safety, and regulatory standards. Maintain competency through continuing education and evidence-based practice. Promote CMH in a positive and professional manner within the facility and community. Perform other duties as assigned. Education, Training, and Experience Bachelor's, Master's, or Doctor of Physical Therapy (DPT) degree from an accredited Physical Therapy program. Current New York State license or eligibility for licensure as a Physical Therapist. Experience in skilled nursing, outpatient rehabilitation, acute care, or hospital rehabilitation is preferred but not required. Knowledge, Skills, and Abilities Knowledge of physical therapy evaluation and treatment principles. Ability to perform direct patient care safely and effectively. Ability to operate and maintain rehabilitation equipment. Strong communication and interpersonal skills. Strong clinical reasoning and problem-solving abilities. Ability to collaborate effectively with an interdisciplinary healthcare team. Excellent organizational and time management skills. Basic computer proficiency and experience with electronic medical records preferred. Commitment to providing compassionate, patient-centered care. Physical Requirements Ability to stand, walk, bend, stoop, kneel, squat, and reach throughout the workday. Ability to safely lift, transfer, reposition, and assist patients using appropriate body mechanics and mechanical lifting equipment. Ability to lift up to 50 pounds independently and greater weights with assistance. Ability to push and pull wheelchairs, stretchers, and rehabilitation equipment. Ability to perform repetitive upper extremity movements and fine motor tasks. Ability to respond appropriately in emergency situations. Clinical Supervision Provides clinical supervision and delegation to Physical Therapist Assistants (PTAs), rehabilitation aides, therapy technicians, students, and ancillary personnel in accordance with New York State regulations and professional standards.

$40.00 - $50.00 per hour
