Job Description

Overview We are seeking an experienced and compassionate PRN Home Health Physical Therapist to join our healthcare team in Southern Lorain County. The ideal candidate will possess a strong background in patient care, with experience in both outpatient and geriatrics settings. This role is crucial in helping patients regain mobility and improve their quality of life through tailored physical therapy programs. The Physical Therapist will collaborate with other healthcare professionals to ensure comprehensive care and effective discharge planning. This is a pay per visit position. With flexibility in scheduling and balancing that work- personal life aspect. PRN position to start with the possibility of PT or FT in the future Duties Conduct thorough assessments of patients' physical abilities and limitations. Develop individualized treatment plans based on patient needs, goals, and medical history. Implement therapeutic exercises and modalities to enhance patient recovery. Educate patients and their families on treatment plans, exercises, and injury prevention. Maintain accurate documentation of patient progress and treatment outcomes. Collaborate with interdisciplinary teams within hospital or clinic settings to optimize patient care. Stay informed about advancements in physical therapy practices and medical terminology relevant to the field. Participate in discharge planning to ensure smooth transitions for patients leaving the facility. Qualifications Doctor of Physical Therapy (DPT) degree from an accredited program. Valid state licensure or eligibility for licensure as a Physical Therapist. Strong knowledge of anatomy, physical therapy techniques, and medical terminology. Experience in outpatient settings preferred; familiarity with geriatric patient care is a plus. Excellent communication skills to effectively interact with patients, families, and healthcare teams. Ability to work collaboratively within a multidisciplinary environment. Commitment to ongoing professional development and education in the field of physical therapy. Pay:
$60.00 - $90.00 per hour
