Job Description

PRN Physical Therapist (PT) - Victorian Assisted Living Overview Join our dedicated healthcare team as a PRN Physical Therapist, where your expertise will play a crucial role in enhancing the recovery and quality of life for our residents across The Victorian. You will evaluate, develop, and implement personalized care plans to promote mobility, reduce pain, and restore functional independence, helping our residents achieve their highest potential within a supportive community setting. We are building an In House Therapy Team and cannot wait to see the positive impact it will bring to the residents of The Victorian. Responsibilities Conduct comprehensive assessments of residents' physical conditions, including mobility, strength, balance, and functional capabilities, within The Victorian. Develop and execute tailored care plans that incorporate evidence-based physiotherapy techniques, such as therapeutic exercise, manual therapy, and gait training, specifically adapted for geriatric populations and those with various age-related conditions. Provide hands-on therapy sessions to improve resident outcomes in areas like geriatrics, orthopedic rehabilitation, neurological rehabilitation, and post-surgical recovery. Collaborate with an interdisciplinary team including nurses, occupational therapists, speech therapists, and facility staff to coordinate care, optimize resident well-being, and facilitate seamless transitions on an off therapy. Educate residents and their families on treatment strategies, fall prevention, assistive device use, and home exercise programs to support ongoing recovery and promote independence within their living environment. Maintain accurate and timely documentation within electronic medical record (EMR) systems, including initial evaluations, progress notes, treatment plans, and discharge summaries, adhering to all regulatory guidelines. Stay current with best practices in geriatric physical therapy, medical terminology, and anatomy knowledge to deliver precise and effective interventions relevant to the resident population. Skills Experience working with geriatric populations, preferably within skilled nursing, assisted living, or long-term care facilities, with a strong understanding of age-related physical needs and conditions. Expertise in care planning for diverse populations, with a focus on patient-centered and resident-centered care, emphasizing functional independence and quality of life. Proficiency in a range of physiotherapy techniques, including therapeutic exercise, manual therapy, balance training, and modalities appropriate for geriatric rehabilitation. Familiarity with therapy discharge planning processes and facilitating safe transitions for residents coming off therapy. Experienced with EMR systems for efficient documentation and resident record management - Specifically Net Health Optima and Point Click Care. Comprehensive knowledge of physiology, anatomy, medical terminology, and the principles of physiotherapy practice, especially as they apply to the older adult. Strong ability to communicate effectively and compassionately with residents, families, and team members, fostering a positive and encouraging therapeutic environment. Ability to adapt therapy approaches to meet the unique needs and cognitive levels of residents varying levels of physical and cognitive function.
