Navy Blue Scrubs Position Summary We are seeking a compassionate and skilled Home Health Physical Therapist (PT) to join our growing team. In this role, you will provide in-home physical therapy services to patients throughout the designated coverage area. The Physical Therapist is responsible for assessing, planning, implementing, and evaluating patient care to improve mobility, functional independence, strength, endurance, pain management, and overall quality of life. Key Responsibilities Perform comprehensive evaluations and assessments of patients' physical therapy needs, including functional status, mobility, strength, endurance, balance, and pain levels. Develop individualized treatment plans based on physician orders, clinical findings, and patient goals. Provide therapeutic interventions designed to improve mobility, restore function, reduce pain, and enhance overall well-being. Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes. Educate patients, families, and caregivers on exercises, safety techniques, disease management, and mobility strategies. Collaborate with physicians, nurses, case managers, and interdisciplinary team members to coordinate patient care. Complete accurate and timely clinical documentation in accordance with agency, state, and federal guidelines. Participate in case conferences and communicate changes in patient condition to the appropriate healthcare team members. Ensure compliance with all regulatory, company, and professional practice standards. Required Qualifications Graduate of an accredited Physical Therapy program. Current, unrestricted Physical Therapist (PT) license. Strong clinical assessment and patient care skills. Valid driver's license, reliable transportation, and ability to travel within the assigned coverage area. Ability to manage a caseload independently and efficiently. Preferred Qualifications Home health experience preferred. Proficiency with HCHB (Homecare Homebase) documentation system. Excellent communication and interpersonal skills. Strong organizational and time management abilities. Ability to build rapport with patients and families while delivering high-quality care.
