Physical Therapist Assistant Green Valley Home Health Services Apple Valley, CA Job Details PRN $40 - $60 an hour 9 hours ago Qualifications Spanish Physical Therapy BLS Certification Care documentation Driver's License Clinical information systems Patient interaction Licensed Physical Therapist Assistant Orthopedics Full Job Description Overview We are seeking a licensed Physical Therapist Assistant to provide skilled, compassionate care in patients' homes throughout the High Desert. Working under the direction and supervision of a Physical Therapist, you will carry out established treatment plans, help patients build strength and mobility, and support safer, more independent daily living. Responsibilities Provide physical therapy treatments delegated by the supervising Physical Therapist and consistent with the established plan of care, provider orders, state law and agency policy. Guide patients through therapeutic exercise, gait and transfer training, balance activities, functional mobility practice and approved physical modalities within the PTA's scope and demonstrated competencies. Observe and measure patient performance and response to treatment; promptly report changes in condition, safety concerns, barriers, questions and lack of expected progress to the supervising PT and appropriate care team members. Reinforce patient and caregiver education on home exercise programs, fall prevention, safe mobility, equipment use and strategies identified in the plan of care. Document each visit, interventions, patient response, communication and progress accurately. Coordinate visit schedules and care communication with the supervising PT, clinical manager and interdisciplinary team; participate in case conferences, in- services and quality activities. Support safe use of assistive or adaptive equipment and communicate equipment needs or problems to the supervising PT. Follow infection prevention, privacy, professional ethics, patient rights and emergency procedures in every care setting. Required Qualifications Graduate of a physical therapist assistant education program accredited by the Commission on Accreditation in Physical Therapy Education or otherwise meeting the licensure requirements of California. Current, unrestricted Physical Therapist Assistant license or certification in California by the start date. Current Basic Life Support certification, or ability to obtain it by the start date. Valid driver's license, reliable transportation and automobile insurance that meet agency and state requirements. Ability to work independently between supervisory contacts, follow an established plan accurately and complete electronic clinical documentation on time. Clear, respectful communication with patients, caregivers, the supervising PT and interdisciplinary colleagues. Ability to meet the essential physical functions described below, with or without reasonable accommodation. Preferred Qualifications Clinical PTA experience; home health, geriatric, neurologic or orthopedic experience is preferred. Experience with Medicare home health requirements and WellSky Home Health EMR. Demonstrated skill in patient and caregiver education, fall prevention and functional mobility training. Spanish language skills.

$40.00 - $60.00 per hour
