We are seeking a dedicated and compassionate Physical Therapist or Physical Therapist Assistant to join our rehabilitation team and provide high-quality therapy services to our residents at Vista Post Acute! We are a 71 bed Skilled Nursing Facility in Hayward, serving short term rehab and long term care patients.
Responsibilities:
Evaluate residents' functional mobility, strength, balance, coordination, and physical abilities as appropriate to the role. Develop and implement individualized treatment plans under the appropriate scope of practice. Provide therapeutic exercises, gait training, balance activities, transfer training, and functional mobility interventions. Assist residents with improving strength, endurance, independence, and safety with daily mobility. Monitor residents' progress and modify treatment approaches as needed in collaboration with the therapy team. Document evaluations, treatments, progress, and other required clinical information accurately and timely. Communicate with nursing staff, physicians, families, and other members of the interdisciplinary team regarding residents' needs and progress. Educate residents, caregivers, and staff on appropriate exercises, mobility techniques, positioning, and safety strategies. Follow all facility, clinical, and regulatory requirements related to patient care and documentation. Promote a safe, respectful, and positive environment for residents.
Qualifications:
Current California PT or PTA license in good standing. SNF, geriatric, or post-acute rehabilitation experience preferred but not required. Strong clinical, communication, and organizational skills. Ability to work effectively as part of an interdisciplinary team. Compassionate, dependable, and resident-focused approach to care.
