Our mission is to ensure a smooth transition for patients returning home from medical facilities by offering expert guidance throughout their recovery journey. As a Physical Therapist Assistant with Best Home Health Providers, you will work alongside a licensed Physical Therapist to help patients recover, regain mobility, and return to the activities they love — all from the comfort of their homes. Please note that this role is per diem with a rate of $95 per visit. Responsibilities Deliver hands-on physical therapy under the direction of a licensed Physical Therapist, following each patient's care plan. Guide patients through therapeutic exercises, gait and transfer training, and the use of assistive devices such as wheelchairs, braces, crutches, canes, and prosthetics. Apply physical modalities including heat, cold, ultrasound, electrical stimulation, and massage to relieve pain and restore function. Monitor patient progress and communicate changes to the Physical Therapist and care team. Document care accurately and on time, and participate in team meetings and ongoing training. Perform other duties as required for patient services Requirements Associate's degree from an APTA-accredited Physical Therapy Assistant program. Active PTA license from the Physical Therapy Board of California. Current CPR certification with AHA. Strong communication and organizational skills. Valid driver's license, reliable insured vehicle, and willingness to travel to patient homes.
