Job Overview We are seeking a dedicated and energetic Home Health Licensed Physical Therapy Assistant (LPTA) to join our growing Privately Owned home health. In this vital role, you will provide high-quality physical therapy services directly to patients in their homes, supporting their recovery and enhancing their quality of life. Your expertise will help implement personalized treatment plans, promote mobility, and facilitate safe discharge planning. Duties Assist licensed physical therapists in implementing comprehensive physical therapy treatment plans tailored to individual patient needs. Conduct clinical assessments and document patient progress accurately following established standards for medical documentation and electronic health records (EHR) management. Apply manual therapy techniques, including trigger point therapy, to alleviate pain and improve mobility. Operate and maintain medical equipment used during therapy sessions, ensuring safety and proper functionality. Collaborate closely with healthcare professionals such as physicians, occupational therapists, and caregivers to coordinate care plans and optimize patient outcomes. Educate patients and caregivers on exercises, mobility strategies, and safety precautions to promote independence at home or in outpatient settings. Assist with discharge planning by providing detailed documentation of patient progress and recommendations for ongoing care or referrals. Qualifications Valid state licensure as a Physical Therapy Assistant (LPTA) with current certification required. Proven experience in home health care, skilled nursing facilities, outpatient clinics, or inpatient settings with a focus on geriatrics, pediatrics, orthopedics, or acute care. Strong knowledge of physiology, anatomy, medical terminology, and clinical assessment documentation standards. Hands-on experience with physical therapy treatment implementation including manual therapy techniques such as trigger point therapy. Familiarity with medical equipment operation and electronic health records (EHR) management systems. Excellent communication skills for effective caregiver and client interaction; ability to explain care plans clearly and compassionately. Ability to work independently in patients' homes while maintaining high standards of clinical documentation and safety protocols. Additional preferred experience includes outpatient or clinic experience, inpatient skilled nursing facility work, geriatrics, pediatrics, orthopedics, and discharge planning. Pay:
$45.00 - $55.00 per hour Expected hours: 35.0 - 45.0 per week
