Renewal Rehab is seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team serving seniors in skilled nursing and post-acute care. In this on-site role, you'll deliver evidence-based treatments under a licensed PT's direction to improve mobility, safety, and independence. You'll manage a manageable caseload, document outcomes, and collaborate closely with an interdisciplinary team. Responsibilities Provide one-on-one physical therapy interventions to senior patients under the direction of a licensed PTImplement individualized treatment plans focusing on mobility, strength, balance, and functional independence Accurately document treatments, progress notes, and outcomes in the EMR in a timely manner Educate patients, families, and caregivers on safe mobility techniques and home exercise programs Collaborate with an interdisciplinary team to support discharge planning and functional goals Maintain compliance with state practice acts, facility policies, and payer/Medicare regulations Contribute to a positive, patient-centered culture built on integrity, respect, and clinical excellence Required Skills Therapeutic exercise implementation Gait training Balance and fall-prevention techniques Transfer and mobility training Therapeutic modalities (e.g., heat, cold, e-stim per state law) Treatment documentation and EMR use Care plan implementation under PT direction Patient and family education Interdisciplinary team collaboration Knowledge of Medicare and skilled nursing regulations
