Summary of job description: The Physical Therapy Assistant is responsible for carrying out treatment, implementation and documentation for home health physical therapy patients under the direction of the registered physical therapist and according to physician's orders.
Essential functions/job duties:
Implements physical therapy services in accordance with the plan of treatment developed by the RPT and signed by the physician. Demonstrates competency in skills required by the agency. Collaborates with the RPT in assisting the physician in assessing the patient's functional level by applying appropriate tests. Assists in the development and revision of the plan of care. Treats patients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care and under supervision of the RPT. Observes, records and reports the patient's reaction to treatment and any change in the patient's condition to the RPT and/or clinical case manager. Directs and aides patients in active and passive therapeutic exercises, muscle reduction, gait and functional training, transfer activities and prosthetic training. Utilizes physical agents, including, but not limited to heat, cold, water, light, electricity, ultrasound and massage. Instructs and advices patients, family and other health team personnel including, when appropriate, home health aides, in the phases of physical therapy in which they may assist the participant; instructs all of the above in the use of prosthetic and orthotic devises and durable medical equipment, as appropriate. Assists in preparing patient clinical notes and progress reports according to agency policy and time frames. Initiates and attends patient care conferences for complex and/or multidisciplinary patients when required or helpful to ensure good coordination of care. Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to agency policies. Completes accurate visit documentation within 24 hours of visit and submits them per agency guidelines.
