$70,000-$102,000 annually

What You'll Be DoingDeliver Exceptional Rehabilitation Services Implement treatment plans developed by licensed Physical Therapists Provide gait training, balance activities, ROM exercises, and mobility interventions Teach exercises, safety strategies, and self-care techniques to patients and caregivers Monitor patient progress and communicate updates to the supervising PT Help patients maximize their independence and quality of life Collaborate Across The Continuum Work alongside Physical Therapists, nurses, and interdisciplinary clinicians Document visits and patient progress using streamlined EMR systems Contribute to outstanding patient outcomes Support patients throughout their recovery journey Required Qualifications Active Minnesota Physical Therapy Assistant license Valid driver's license, insurance, and reliable transportation Strong communication and relationship-building skills Passion for patient-centered care Preferred Qualifications Home health experience Experience with geriatric or medically complex patients Desire to work independently while being part of a collaborative team Great Therapy Is About More Than Movement.
