Home Health Physical Therapist Assistant

Job Description

Provide skilled Physical Therapy services, on a intermittent basis, to patients, in their homes, in accordance with physician orders Under the supervision of the physical therapist, assist in evaluation and development of a rehabilitative plan of care and in periodic re-evaluation, as required As directed, perform routine strengthening, and therapeutic, exercises and assist the patient with the use of self-help devices. Observe, and record, activities and findings, in the clinical record, and reports to the PT and the physician the patient's reaction to treatment and any changes in the patient's condition Write daily progress notes on patients; submit same, within 72 hours, and prepare periodic summary reports, as indicated. Summary As a Physical Therapist Assistant, you will be instrumental in delivering high-quality patient care under the supervision of a licensed physical therapist. This role requires a strong foundation in anatomy, physiology, and medical terminology, along with specialized skills in physical therapy techniques. You will engage with patients across various settings, including outpatient clinics, skilled nursing facilities, and home health environments. Your expertise in discharge planning and care plans will ensure effective rehabilitation outcomes, particularly in orthopedics and geriatrics. Qualifications Strong patient care skills and knowledge of physiology and anatomy Proficient in medical terminology and physical therapy techniques Experience in various settings including inpatient, outpatient, and skilled nursing facilities Familiarity with discharge planning and care plans Knowledge of trigger point therapy and experience with orthopedics and geriatrics
