PTs will:
Evaluate mobility, strength, balance, endurance, and functional abilities Develop individualized treatment plans based on resident needs and goals Provide therapeutic exercise, gait training, transfer training, balance activities, and functional mobility interventions Help residents regain independence following illness, injury, surgery, or hospitalization Identify and address fall risk, mobility decline, weakness, and changes in function Educate residents, families, and caregivers on treatment goals and safety strategies Collaborate with nursing, physicians, occupational therapy, speech therapy, and the interdisciplinary team Participate in care planning and advocate for residents' functional needs Monitor progress and adjust treatment plans to maximize outcomes Complete timely and accurate documentation in accordance with Medicare, Medicaid, state, federal, and facility requirements.
PTAs will:
Improve walking, balance, strength, and endurance Practice safe transfers and functional mobility Participate in therapeutic exercise and gait training Reduce fall risk and improve confidence with movement Regain function following illness, injury, surgery, or hospitalization Maintain mobility and independence as they age Learn safe techniques and strategies that support daily function Working under the direction of a Physical Therapist, you will carry out individualized treatment plans, monitor resident response to treatment, communicate changes in function, educate residents and caregivers, and collaborate with the interdisciplinary team. What We're Looking For We're looking for someone who brings clinical skill, compassion, teamwork, and a genuine desire to make a difference.
Qualifications include:
Current, unrestricted Physical Therapy or Physical Therapy Assistant license in South Carolina Current CPR certification Strong clinical assessment and treatment-planning skills Excellent communication and interpersonal skills A compassionate, resident-centered approach to care Ability to work effectively with geriatric and medically complex populations New Graduates Are Welcome! Experience in skilled nursing, long-term care, rehabilitation, or geriatrics is preferred, but it is not required. We welcome experienced clinicians and new graduates who are passionate about working with older adults and want an opportunity to grow in a supportive environment. Knowledge of Medicare Part A and Part B rehabilitation services, electronic medical records, and geriatric rehabilitation is a plus.
