PRACTICE

As a Primary Care Physician, you will provide comprehensive outpatient care to adults, with a strong focus on the Senior/Medicare Advantage population . The practice is built around a relationship-driven model that allows physicians to focus on: Preventive care Chronic disease management Early intervention Coordinated care Patient education Long-term health outcomes This is NOT a high-volume, "see as many patients as possible" environment. With an average of approximately 15 patients per day , you'll have the opportunity to actually know your patients and make a meaningful difference in their lives.

RESPONSIBILITIES

Provide comprehensive outpatient primary care to adult and senior patients Develop long-term relationships with a smaller patient panel Manage chronic conditions and provide preventive care Collaborate with an interdisciplinary care team Deliver high-quality, patient-centered, value-based care Help improve outcomes for the Medicare Advantage population

QUALIFICATIONS

MD or DO Board Certified or Board Eligible in Internal Medicine or Family Medicine Active Florida medical license Value-Based Care experience preferred Experience with the Senior/Medicare Advantage population is a plus Geriatric Fellowship graduates are encouraged to apply Passion for compassionate, relationship-driven patient care
