Job Description Help for Partial Job Description. Opens a new window. Conviva Senior Care Centers is seeking a Primary Care Physician for a position near Jacksonville in Callahan, Florida, with total earnings up to $300k plus sign-on bonus and relocation.

• See 12-15 patients per day. Opportunity to spend more time with patients
• Thrive in a value-base care model and improve patient's quality of life
• See a variety in your caseload, from preventative care to chronic conditions
• Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care Required Qualifications
• Doctor of Medicine or Osteopathic Medicine
• Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
• Valid, unrestricted license in the state of desired employment (or willing to obtain)
• Current, unrestricted DEA license
• Screened for TB
• Minimum of two+ years of experience in value base care
• 1+ years of experience managing Medicare Advantage panel of patients
• Associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency.

Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government. #LI-FM1 Your Accomplishments are

• Doctor of Medicine or Osteopathic Medicine
• Board certified or Board certified in Geriatric Medicine, Family Medicine or Internal Medicine (We recognize the American Board of Physician Specialties® (ABPS) Certification)
• Minimum of two to five years directly applicable experience preferred
