Internal/Family Medicine (Primary Care) Collaborating & Supervising Physician
Job Description
The Internal/Family Medicine Collaborating & Supervising Physician is a board-certified Internal or Family Medicine physician who serves as the clinical collaborator and supervising physician for Family Nurse Practitioners (FNPs) within Vivia Health's Primary Care program. This role is primarily supervisory and consultative in nature, focused on clinical oversight, mentorship, quality assurance, and regulatory compliance. The physician supports safe, effective, and evidence-based care delivery while advancing Vivia Health's mission of community-based, patient-centered healthcare. This position is well suited for a physician educator with a passion for mentorship and team-based care. Physicians actively operating an independent private practice are not eligible for this role. Key Responsibilities 1. Clinical Supervision & Collaboration
+ Provide clinical supervision and collaborate with approximately 0.5 to 1.5 FTE (1-3 total) Family Nurse Practitioners across up to three clinic locations.
+ Support FNPs in diagnosing, treating, and managing patients in accordance with evidence-based practices and organizational protocols.
+ Conduct periodic chart and case reviews, including approvals of treatment plans as required.
+ Provide guidance on primary care services, clinical workflows, preventative medicine, and best practices.
+ Promote collaborative, integrated care by working closely with other Vivia providers, including psychiatry and behavioral health, to support coordinated treatment planning and continuity of care. 2. Physician Oversight & Leadership Support
+ Serve as a clinical leader for the Primary Care program, advising leadership on clinical quality, risk management, and service delivery.
+ Provide escalation support for complex or high-risk clinical cases and coordinate with department supervisors on internal referral workflows to help ensure timely, appropriate handoff and follow-up across services.
+ Participate in training and education initiatives to support continuous professional development of FNPs and care team members. 3. Time Commitment & Availability
+ Estimated time commitment of approximately 2-6 hours per week, depending on clinical needs and staffing levels.
+ Participate in a minimum of weekly supervision meetings (in-person or remotely) with each supervising FNP.
+ Attend at least two (2) management meetings (in-person or remotely) per month (approximately one hour each).
+ Maintain availability by phone, text, or email during clinic operating hours for consultation.
+ Complete a minimum of one (1) hour of on-site presence per month. 4. Documentation, Compliance & Quality Assurance
+ Ensure collaborative and supervisory practices comply with all applicable federal and Illinois state regulations.
+ Document supervision activities, chart reviews, and clinical guidance in accordance with organizational policy and payer requirements.
+ Support audit readiness through timely, accurate, and compliant documentation.
+ Contribute to quality assurance efforts and continuous improvement of patient care services. 5. Direct Patient Care (As Needed)
+ Provide direct patient care on an as-needed basis for complex cases, coverage, or patient safety needs.
+ This role does not carry a standing patient panel and is primarily supervisory in nature.
+ Model patient-centered, trauma-informed, and culturally responsive care practices. Qualifications Required +
Education:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school. +
Licensure:
Active, unrestricted medical license in the State of Illinois. +
Board Certification:
Internal Medicine or Family Medicine. +
Experience:
Minimum of 5-7 years of clinical experience in Internal or Family Medicine.
+ Demonstrated experience supervising or collaborating with Nurse Practitioners (preferred).
+ Strong working knowledge of state and federal regulations governing NP collaboration and supervision. Skills & Competencies

+ This position is intended for physicians who are not currently operating an independent private practice.
